Role Description The Physical Therapist is a full-time, on-site role based at St. James Hospital in Hornell, NY. This position is responsible for evaluating patients, developing individualized treatment plans, and providing evidence-based therapeutic interventions to support recovery, mobility, and functional independence. Day-to-day responsibilities include collaborating with physicians and other care team members, documenting patient progress, educating patients and families on home exercise programs, and ensuring safe discharge planning. The Physical Therapist will work across inpatient and outpatient settings as needed, contribute to quality improvement initiatives, and uphold hospital standards for patient safety and satisfaction. Qualifications Demonstrated expertise in Physical Therapy and Rehabilitation, with the ability to design and implement effective treatment plans. Strong skills in Patient Care and Home Care, including patient education, functional training, and support for safe transitions. Foundational knowledge of Medicine relevant to physical therapy practice, including musculoskeletal, neurological, and cardiopulmonary conditions. Current state license (or eligibility for licensure) as a Physical Therapist; Doctor of Physical Therapy (DPT) or equivalent required. Excellent communication, interpersonal, and documentation skills, with the ability to work collaboratively in a multidisciplinary team. Ability to prioritize caseloads, manage time effectively, and maintain professional integrity and ethical standards.
