Pay: $82,000–$92,000 per year + UNCAPPED COMMISSIONS + Performance-Based Incentives & Bonuses

A Day in the Life of a Perfect Care Match Community Relations Case Manager

No two days will look exactly the same. This is a field-based, relationship-driven growth role for someone who enjoys being in the community, developing business, helping families, and producing measurable results.

Your day may include:

• Building and developing referral relationships with hospitals, rehabilitation centers, senior living communities, physicians, social workers, elder law attorneys, geriatric professionals, and other referral partners

• Prospecting and opening new referral opportunities throughout the South Shore

• Conducting in-person meetings, presentations, networking, and community outreach

• Following up quickly on new referrals and prospective clients

• Helping families understand their care options, providing the operations team with all details needed for services

• Coordinating new case starts with our care, clinical, recruiting, scheduling, and operations teams

• Following up with existing clients, families, and referral partners

• Maintaining accurate CRM, referral, pipeline, and communication records

• Tracking referrals, conversions, case starts, census growth, and territory performance

• Identifying new market opportunities and sharing insights with leadership

You should expect to spend a significant portion of your time on the road throughout the South Shore building relationships and developing business.

Success in this role is not measured simply by activity. It is measured by your ability to turn relationships into qualified referrals, new clients, census growth, and long-term partnerships.

• Base Salary: $82,000–$92,000 per year , based on experience, qualifications, industry relationships, and demonstrated performance
