Nurse Practitioner or Physician Assistant - Outpatient Primary Care

Job TitleThis job is an integral part of the care delivery system working together to provide a healing environment, by placing our customers first. The incumbent provides primary healthcare services for non-acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization within the CRNP scope of practice. Work is performed in a physician office or offices dedicated to Primary Care, weekend, and on-call coverage on a rotational basis, as required.Job ResponsibilitiesThe essential responsibilities include:Providing appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, providing accurate medical record documentation.Communicating and coordinating patient care with patient, family, community health services and interdisciplinary team.Providing monitoring and continuity of care and coordinating periodic evaluation by physicians as defined in the written agreement between the CRNP and the supervising physician(s).Preparing written prescription orders for drugs and controlled substances, administering medication and injections, suturing wounds and providing wound care.Educating patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions and recommending community resources as appropriate.Triaging patient telephone calls, providing monitoring and continuity of care.Participating, developing, and implementing performance and/or quality improvement initiatives. Ensuring compliance with all regulatory agencies.May perform other duties such as manage outpatient concerns and any other duty specific to specialty.Other duties as assigned or requested.QualificationsRequired:Master's degree MSN from the practicing State Board of Nursing Approved Master's Program.National Board Certification for Nurse Practitioners through an accredited association.Practicing-state licensure as a Nurse Practitioner.Additional Employment Requirements:Prescriptive Authority from the practicing state Within 60 days of hire.Drug Enforcement Agency Licensure Within 60 days of hire.CPR – American Heart Association.Act 34 Criminal Background Clearance Certificate.Act 33 Child Abuse Clearance Certificate.Act 73 FBI Fingerprinting Criminal Background Clearance Certificate.
