Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist is responsible for evaluating all assigned patients and developing, implementing, and modifying treatment plans in accordance with the patient's needs, goals, and physician directions. This role includes delegating and directing all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate effective return to community activities. The Physical Therapist serves as an educator for patients, families, students, and coworkers while upholding the organization’s Mission Statement and Core Values.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop treatment plans and goals aimed at progressing patients towards discharge within an established length of stay.
• Document medical records accurately and promptly, including goal writing, functional quality indicators, and letters of medical necessity.
• Participate in student programs as assigned, providing a positive learning environment with supervision and evaluations.
• Create an individualized professional development plan in collaboration with clinical management.
• Adhere to discipline-specific State Practice Acts and maintain professional licensure in good standing.
• Engage in organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for the practice of Physical Therapy.

Schedule

This is a 13-week contract position with a 5x8.0 schedule.
