Job Summary:
As an On-Call Registered Nurse, you'll provide after-hours clinical support to Participants, caregivers, and authorized representatives with urgent and non-urgent needs. You'll use your nursing knowledge, clinical judgment, and understanding of the frail older adult population to assess concerns, provide appropriate guidance, coordinate care, and connect Participants with the right resources.
This position serves as an important after-hours clinical triage and coordination point. You'll work independently while having access to designated providers, clinical leaders, and other escalation resources. You will help ensure that Participants receive safe, timely support while keeping the responsible center and care team informed for follow-up.
This position is ideal for someone who is calm under pressure, confident in their nursing judgment, organized, and comfortable managing multiple priorities. You'll be part of a team that values participant-centered care, communication, safety, and continuity of care.
Schedule:
Remote, after-hours on-call coverage during assigned shifts. The nurse must remain available, responsive, and able to access required systems and clinical resources throughout the assigned coverage period.

Duties/Responsibilities:
- Answer the designated on-call line during assigned coverage periods.
- Identify the Participant, caller, location, reason for the call, and urgency of the concern.
- Complete focused nursing assessments based on the Participant's symptoms, condition, history, medications, recent services, functional status, cognition, caregiver support, and other relevant information.
- Recognize changes in condition, potential emergencies, falls, injuries, medication concerns, behavioral or cognitive changes, and other risks requiring immediate action or escalation.
- Provide clear and safe nursing guidance within the Registered Nurse scope of practice, approved protocols, the Participant's Plan of Care, and available provider orders.
- Use teach-back when appropriate to confirm that Participants and caregivers understand the guidance provided.
- Direct callers to emergency services or activate the appropriate emergency response pathway when emergency evaluation is needed.
- Place Participant safety, preferences, goals, dignity, and individual needs at the center of each decision.
- Work with Participants, caregivers, and authorized representatives to develop an appropriate next step while maintaining privacy and appropriate consent practices.
- Identify barriers such as transportation, medication access, caregiver availability, home environment, health literacy, or access to services and escalate concerns as needed.
- Coordinate appropriate next steps, which may include monitoring, self-care guidance, provider consultation, pharmacy coordination, home-based services, center follow-up, urgent care, emergency department evaluation, or emergency medical services.
- Escalate concerns to the appropriate provider, medical director, clinical leader, center team, pharmacy, home care, transportation, behavioral health, or other designated resource.
- Communicate clear and complete clinical information to the appropriate team using established handoff processes.
- Notify the responsible center and clinical team of calls requiring follow-up, intervention, reassessment, order execution, care plan review, or Participant outreach.
- Confirm that urgent handoffs have been received by the appropriate person or team and re-escalate unresolved high-risk concerns when necessary.
- Support continuity of care across OSC centers and states by accurately identifying the Participant's enrolled program, center, current location, and responsible follow-up team.
- Document all Participant-related calls, assessments, interventions, consultations, escalations, dispositions, and follow-up directions in the designated electronic medical record or approved system.
- Complete documentation as soon as possible and before the end of the assigned on-call coverage period whenever possible.
- Ensure documentation is objective, accurate, clinically meaningful, and sufficient to support continuity of care.
- Document the reason for the call, assessment findings, nursing judgment, guidance provided, individuals contacted, orders or instructions received, disposition, Participant or caregiver understanding, and required follow-up.
- Document unsuccessful contact attempts, communication barriers, refusal of recommendations, deviations from the established plan, and actions taken to reduce risk.
- Complete required incident, adverse event, medication event, fall, hospitalization, emergency department, and other reports according to OSC policy.
- Provide a reliable handoff to the responsible center, clinical, provider, and interdisciplinary teams following an event.
- Identify pending actions, recommended reassessment, medication or order needs, outside services used, Participant or caregiver concerns, and the person or team responsible for follow-up.
- Identify events requiring same-day outreach, post-emergency follow-up, transition-of-care activity, medication reconciliation, diagnostic review, provider evaluation, home assessment, or Plan of Care consideration.
- Participate in case discussions, follow-up reviews, audits, quality improvement activities, and process improvement initiatives as requested.
- Follow OSC policies, on-call protocols, applicable Nurse Practice Acts, professional standards, and established emergency and escalation procedures.
- Maintain knowledge of center contacts, provider coverage, pharmacies, emergency resources, home care, transportation pathways, and state-specific requirements needed for on-call coverage.
- Complete required orientation, state-specific training, annual competencies, emergency response education, privacy and security training, electronic medical record training, and continuing education.
- Identify trends and opportunities for improvement related to after-hours calls, avoidable utilization, access barriers, adverse events, and handoff reliability.
- Report technology, telephone, system access, staffing, or workflow issues through the appropriate escalation process.
- Follow approved downtime procedures when systems or technology are unavailable.
- Maintain confidentiality and conduct calls in a private environment appropriate for protected health information.
- Perform other duties as assigned.

Education and Experience:
- Graduate of an accredited school of nursing.
- Current, active, unrestricted Registered Nurse license in every state in which the nurse is assigned coverage.
- Must obtain and maintain all required state licenses before providing nursing services in that state.
- Minimum of one year of professional nursing experience.
- Experience with frail older adults, geriatrics, PACE, home-based care, triage, emergency or urgent care, primary care, or care coordination is strongly preferred.
- Current Basic Life Support/CPR certification.
- Ability to meet applicable state licensure, background, exclusion screening, competency, health clearance, and continuing education requirements.
Requirements:
- Ability to work assigned after-hours on-call shifts and remain available and responsive throughout the assigned coverage period.
- Ability to access required electronic systems, clinical resources, and communication tools while on call.
- Ability to communicate clearly by telephone and secure electronic methods.
- Ability to complete timely and accurate clinical documentation.
- Ability to maintain a private work environment appropriate for protected health information.
- Ability to perform the essential functions of the position with or without reasonable accommodation.
- Must be able to drive a motor vehicle when required by assignment.
Physical Requirements:
- Ability to sit, stand, bend, reach, walk, and move intermittently throughout the workday.
- Ability to communicate by telephone and secure electronic methods and hear and understand callers with or without reasonable accommodation.
- Ability to use a computer for sustained documentation, electronic medical record navigation, and communication.
- Ability to use OSC-provided or approved equipment, including a secure telephone, computer, multi-factor authentication, and communication applications.
- Ability to travel and provide direct Participant care when required by assignment.
