Physical Therapist (PT) - Rantoul Outpatient Clinic

Overview Evaluates and treats patients of all ages, modifying plan of care as appropriate. Serves as a role model in delivery of professional services and as a clinical resource for staff and students. This role qualifies for a sign-on bonus! Qualifications Education level: Bachelor’s Degree in Physical Therapy Licenses/Certifications: Licensed Physical Therapist (PT) – Illinois Department of Financial and Professional Regulation (IDFPR) Basic Life Support (BLS) within 30 days – American Heart Association (AHA) Continuing education: attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients. Responsibilities Essential Functions Demonstrate ability to perform patient transfers with assistance as needed from bed, chair, and floor, if applicable. Be aware of and demonstrate safe and appropriate body mechanics to reduce the risk of injury to self, staff, and patients. Demonstrate respect by treating patients and each other with dignity and respect, being empathetic and caring, and staying fully engaged with those who need services. Design an appropriate plan of care that integrates goals, treatment, outcomes, and discharge plan; adjust care based on patient response and available evidence. Educate patients, families, caregivers, students, and other health care providers using relevant and effective teaching methods. Evaluate clinical findings to determine physical therapy diagnoses and outcomes of care. Perform examination, selecting reliable and valid examination methods relevant to the chief complaint, results of screening, and history. Produce documentation in accordance with the Clinical Process Policy (CTSCP67). Screens patients using procedures to determine effectiveness of and need for therapy services, if appropriate. Use support personnel according to legal standards and ethical guidelines. Monitor the care delivered and provide regular feedback. Complete annual education as assigned to comply with DNV Hip and Knee Replacement Program requirements, if applicable. Department Specific Job Function Evaluate patients’ impairments related to orthopedic and neurologic conditions. Establish a plan of care based on functional deficits and patients’ goals. Complete objective testing for range of motion, strength, and performance of daily functional tasks. Complete daily documentation and charging in therapy EMR. Apply tape for stabilization, facilitation, or inhibition of movement. Complete therapeutic modalities as appropriate, including ultrasound, interferential, iontophoresis. Communicate with referring providers on patient progress and treatment recommendations. Compensation and Benefits The compensation range for this position is $36.74 per hour to $63.19 per hour. The actual compensation offered to a candidate will depend on experience, qualifications, location, training, licenses, shifts worked, and compensation model.
