Therapist - Physical Therapist

Job Summary:

Provide expert physical therapy services in a home health setting, focusing on patient assessment, treatment planning, and supervision of support staff to promote recovery and independence.

Responsibilities:

• 
  Prioritize and coordinate start of care visits to ensure timely patient service.
  
  
• 
  Supervise and oversee the performance of Physical Therapy Assistants.
  
  
• 
  Complete and submit clinical notes and documentation within 48 hours of each visit.
  
  
• 
  Communicate effectively with supervisors, intake teams, and office staff to ensure seamless care delivery.
  
  
• 
  Conduct day-after reporting of start of care visits as required.
  
  
• 
  Maintain compliance with all documentation, licensing, and practice standards.
  
  

Qualifications:

• 
  Hold an active California PT license.
  
  
• 
  Possess at least one year of experience in home health therapy.
  
  
• 
  Demonstrate strong clinical assessment and documentation skills.
  
  
• 
  Be responsive and communicative during standard business hours.
  
  
• 
  Ability to supervise and collaborate with PT Assistants and team members.
