Job Title Physical Therapist Location Texas | Outpatient Schedule / Job Type Contract | Days | 40 hours per week | 13 weeks Key Responsibilities Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction Conduct 60-minute evaluations and treatments, ensuring care is provided under signed, valid physician orders and within insurance approvals Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines Delegate and direct discipline-specific therapy services for assigned patients, designing functional treatment programs that support effective return to community activities Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned, providing appropriate supervision and completing student evaluations Adhere to discipline-specific State Practice Act and all regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements Required Qualifications Active Physical Therapist license in good standing (Texas licensure required) Compliance with all state-mandated continuing education requirements Ability to complete comprehensive patient evaluations and develop evidence-based treatment plans Proficiency in medical record documentation in accordance with state, federal, and payer guidelines Commitment to patient and employee safety standards and organizational compliance requirements Preferred Experience / Skills Prior experience in an outpatient physical therapy setting Experience with functional quality indicator assessments and letters of medical necessity Involvement in clinical student programs or mentorship of therapy students Why Join Us Opportunity to work in a structured outpatient environment with a focus on patient-centered, goal-driven care Supportive setting that encourages ongoing professional development and continuing education Concentric Healthcare Staffing Job ID #778772.
