Job Title: Full Time Registered Nurse Care Manager

Stress Care of New Jersey, LLC is a Behavioral Health and Addiction Treatment Center licensed by the State of NJ and accredited by The Joint Commission. At Stress Care, we provide a full range of Outpatient Services, including Psychiatric Evaluations, Medication Management, Traditional Therapy, Group Therapy, Intensive Outpatient Programs, Partial Care, and Case Management. We work with adults and adolescents and our integrated treatment team consists of Psychiatrists, Nurse Practitioners, Clinical Social Workers, Counselors, Case Managers, Peer Professionals, and other allied team members.

We are currently seeking a Full Time Registered Nurse Care Manager to provide direct health promotion services, referral and linkage to community services, and assist in the implementation of the Care Plan for each recipient and their family enrolled in the Stress Care of NJ BHH/ISC.

Stress Care of NJ BHH strives to integrate primary health care and behavioral health and addiction treatment through a system of comprehensive care coordination in conjunction with the consumer's primary and community support system to ensure that the consumer receives whole health care necessary for successful illness self management.

Qualifications and Skills

• NJ RN License
• Must possess excellent communication skills (written; spoken); excellent organizational, problem-solving, and decision-making skills.
• Ability to work independently and with a team.
• Experience in treatment of psychiatric disorders preferred

Responsibilities and Duties

1. Coordination and supervision of the BHH team.

2. Provides comprehensive care management services through nursing assessment including monitoring health risks by providing screening, preventative care, and early intervention services, analyzing lab/screening reports, and linking to community providers where needed.

3. Leading the BHH team in the management of consumer care and the implementation of the service plan.

4. Development and periodic revision of service plans based on information collected through the consumer assessments, review of consumer records, and input from consumer and family.

5. Ensuring implementation of the plan; to provide and coordinate access to high quality health care services that are informed by evidence-based practices, coordinate access to preventative and health promotion services, coordinate behavioral health services, coordinate specialty medical care, and social services. Plan will include consumer goals and preferences, targeted outcomes, identified service provider, coordinator of services and timeframes for services.

6. Engages consumers in monitoring health and wellness indicators, including blood pressure, blood glucose, lipid profile, weight, waist circumference, BMI, and others.

7. Convening and leading team meetings with BHH team to review and revise consumer service plan periodically and as needed in response to consumer request or other qualifying event, using consumer information and clinical data to monitor adherence to treatment guidelines and best practices for key health indicators.

8. Implementing an internal Quality Assurance program that aligns with organizational required quality measures.
