Physical Therapist Rehabilitation

Requirements

• Current State License as a Physical Therapist - Required
• Proof of current CPR certification - Required
• Current driver's license. Must have reliable transportation and insurance - Required
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
• Previous home care experience - Preferred
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required

Responsibilities

• Perform clinical assessments to identify patient needs and appropriateness of care.
• Document, prepare/review and complete a Plan of Care (POC), progress notes, and other clinical record documentation on an ongoing basis.
• Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders.
• Assist/instruct patients and families in transfer techniques and the safe use of equipment.
• Provide patient and family education regarding therapeutic procedures tailored to individual needs.
• Apply infection control concepts and maintain patient safety during care activities.
