Provides diagnosis and treatment of patients within the scope of practice as defined by all applicable Federal, State and local laws, regulations, protocols, etc. and within the individual Advanced Practice Provider's (APP) particular clinical specialty area, licensure and competency level. Performs acts of medical diagnosis or prescription of medical therapeutic or corrective measures as appropriate.

• Seeking a Family Nurse Practitioner (FNP) or Physician Assistant (PA) to support a Community Health Center dedicated to improving access to care in York County.
• Provide continuity of care for an existing and growing patient population.
• Manage and support an established patient panel, including individuals receiving Medication?Assisted Treatment (MAT).
• Help establish care for a vulnerable and underserved patient population in a mission?driven outpatient setting.
• Deliver both in?person and virtual primary care services, supporting flexible access for patients.
• Collaborate within a supportive care team consisting of four APPs and two Pediatricians.
