We are currently seeking an experienced: HOSPICE & PALLIATIVE CARE NURSE PRACTITIONER Part Time (Covering Bladen County) Job Description: * Implements palliative care program requirements * Completes medical visit with patients per guidelines and coordinate care with Palliative Care team as clinically indicated * Conduct face to face encounter to provide re-certification information to Medical Director/Associate Medical Director for Hospice Patients * Provide clinical findings of that face to face encounter Primary care physician, and/or supervising Medical Director/Associate Medical Director * Communicate with patients/families at all levels. * Communicate with all agency personnel Job Requirements: * Master's degree in Nursing required, with current licensure in the state of practice * Certification as an Adult or Family Nurse Practitioner * Experience in Hospice or Palliative Care nursing preferred * Knowledge of the framework, function, and organization of a hospice care program. * Excellent communication and organization skills.
