Lead Navigator: Housing & Recovery Outreach

Lead Navigator Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona, including residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services for individuals experiencing opioid use disorder, homelessness, and mental illness. Job Summary A Lead Navigator is an individual who has been in recovery from AOD use and/or GMH for at least one year and has at least 90 days of experience as a Navigator or Peer Support Specialist. The Lead Navigator works primarily with homeless individuals, providing outreach, engagement, case management, and ongoing wrap‑around supportive services to help them end homelessness. The Lead Navigator coordinates and monitors community‑based services, serves as a mentor to Navigator II staff, and collaborates with a multidisciplinary team, including Registered Nurses, Counselors, and Clinical Leads. Responsibilities

• Conduct outreach and engagement with high‑acuity, high‑vulnerability individuals.
• Coordinate care and services, including housing referral, leasing, and wrap‑around services.
• Mentor and onboard Navigator II staff.
• Work as part of a multidisciplinary team to coordinate care with community providers and stakeholders.

Qualifications High school diploma or GED required; Associate’s degree or higher in a behavioral health field preferred. Minimum of one year of recovery from substance abuse and/or mental health disorders. 1–3 years of full‑time health‑care related experience preferred. 1–3 years of substance‑abuse/mental‑health recovery‑oriented service experience preferred. Equivalent combination of education and experience acceptable. Current Arizona driver’s license in good standing. Clear 39‑month motor‑vehicle record. Behavioral Health Technician certification in accordance with A.A.C. R9‑10‑115 and CBI Clinical Policies and Procedures. Arizona Fingerprint Clearance card. Benefits Generous PTO accrual (5 weeks).
