If you want to make a difference, we want to hear from you./ph3Job Summary/h3pThe Licensed Practical Nurse (LPN), under the supervision of the Professional Registered Nurse and/or the Physician, provides age-appropriate, culturally and ethnically sensitive care, maintains a safe environment, educates patients and their families, collects information, communicates effectively, and utilizes technology. Participates in the assessment, planning, implementation, and evaluation of the delivery of health care services and tasks. The role of the LPN also includes patient advocacy, teaching, performance improvement, and supervision of unlicensed personnel./ph3Responsibilities/h3ulliParticipates in the collection and analysis of data related to care delivery and reports critical findings to the Registered Nurse and/or Physician./liliParticipates in the development of the individualized plan of care that identifies strategies and alternatives to attain expected outcomes and collaborates with other members of the healthcare team as appropriate./liliParticipates in the execution of the individualized patient/family plan./liliParticipates in the evaluation process toward achievement of outcomes./liliAdvocates for the patient/family rights and educates patient/family concerning their responsibilities./liliProvides education to patient/family and other health care workers./liliParticipates in continuous performance improvement and provides direct personal patient observation, care, and assistance./liliSupervises unlicensed personnel./liliAdditional duties as requested/li/ulh3Qualifications/h3ulliGraduate from an accredited school of Nursing as a Licensed Practical Nurse required/liliCurrent Georgia License as a Practical Nurse/liliCurrent American Heart Association Basic Life Support (BLS) Health Care Provider Card required./li/ulh3Core Competencies/h3pThese competencies reflect the values and behaviors expected of all Grady team members, regardless of role.
