Position Summary

Reports To: Director of Health Information Management

Summary:

Responsible for maintaining patient medical records, prompt and appropriate release of patient information, customer service both in person and on the phone, along with additional administrative duties. Must examine and analyze all medical records to ensure they are correct, accurate, and complete. Accountable for keeping all patient records confidential.

Skills and Abilities

Education: High School Graduate or General Education Degree (GED)

Experience: One to two years of related experience

Computer Skills: Proficient in Microsoft Office, email, and basic computer skills for other software used throughout the hospital for your area of responsibility. Experience with electronic health records is a plus.

Must exhibit critical thinking skills, effective communication skills, and the ability to work with others as a team.

Other Requirements: May take 3 months to become proficient in this position. This position requires frequent standing, walking, sitting, and use of hands and fingers.

Essential Functions:

• Analyze and process all paper/electronic, inpatient/outpatient, admission/discharge documents according to policies on chart requirements and append any necessary deficiencies.
• Continue to work all accounts on the hold work lists and follow up with other departments to ensure the record is complete when issues arise, and in a timely manner.
• Enter deficiencies within the computer system.
• Once a week, process delinquency reports by physicians and send notices via the EMR to the providers.
• Prepare suspension letters if applicable.
• Check for completeness of charts (re-analyze and remove deficiencies from the computer system for those that are a manual process).
• Process all releases of information received in the department electronically and via mail.
• Answer phones and fax medical information to customers both internally and externally.
• Release information to customers, in person, as needed. Do so according to state and federal regulations to ensure confidentiality is maintained.
• Maintain and operate health record indexes to collect, classify, and store information.
• Work to maintain a clean master patient index to avoid duplication of medical record numbers or misspellings. Do so by use of reports and communication with other hospital departments.
• Cross Trained in other areas and uses this flexibility within the department.
• Prepare all charts for scanning and perform the scanning process thru the provided policy and procedure on prepping, scanning, and indexing medical records.
• Process all documents, from all sites, on the closed chart work list daily. Documents will either be inserted or printed and returned to the originator if, after investigation, it is found to be tied to the incorrect account or incorrect patient. All documents will be addressed daily and investigated by staff further if needed, prior to inserting in the record.
