What You’ll Do Step into territories needing support due to census changes or coverage gaps. Provide skilled nursing care, assessments, and care planning. Complete OASIS visits and coordinate interdisciplinary care. Help maintain continuity and quality across teams. Educate patients and families while building trust quickly. Travel throughout the metro with flexibility and independence. What We’re Looking For Strong clinical judgment and ability to adapt quickly in new environments.
