About the Role Licensed Practical Nurse (LPN) – Provide compassionate care to residents. Responsibilities Provide direct patient care, administer medications, monitor vital signs, and perform routine tasks. Assess patients' conditions, identify areas of concern, and report to the healthcare team. Develop and implement individualized care plans in collaboration with interdisciplinary team. Monitor and maintain accurate medical records, ensuring timeliness and accuracy. Maintain a clean and organized environment, adhering to infection control protocols. Collaborate with other healthcare professionals to ensure seamless care transitions. Participate in quality improvement initiatives. Requirements Licensed Practical Nurse (LPN) license in New Jersey. Strong communication, interpersonal, and teamwork skills. Ability to work effectively in a fast-paced, dynamic environment. Compassionate and empathetic approach to patient care.
