Liberty Ln.,Phoenix, AZ, 85048,United States Base Pay $18.88 - $20.77 / Hour Job Category Valley of the Sun YMCA Employee Type Part Time Hourly The Y-OPAS Client Coordinator supports older adults in maintaining independence, connection, and quality of life within their homes and communities. This position conducts client intake and needs assessments, coordinates support services and volunteers, facilitates social activities, and connects clients and families with community resources. The Client Coordinator builds strong relationships with clients, volunteers, families, and community partners while supporting the safe, effective, and compassionate delivery of Y-OPAS services. RESPONSIBILITIES OF A YOPAS CLIENT COORDINATOR: BE WELL Support Safety, Independence & Well-Being Conduct in-home client intake and comprehensive needs assessments to identify physical, social, emotional, and support needs. Monitor client concerns and changes in physical or cognitive abilities and coordinate appropriate follow-up. Schedule regular client follow-ups and assist with connecting clients to appropriate support services. Maintain accurate and timely client records, case notes, and documentation. Support client safety, confidentiality, and compliance with YMCA policies and procedures. Provide care coordination support in partnership with families, volunteers, and community service providers. DO GOOD Build Relationships & Strengthen Community Connections Build trusting relationships with clients and families while providing compassionate, respectful support. Recruit, engage, and support volunteers who provide transportation, shopping, errands, friendly visits, and other services. Provide ongoing guidance and communication to volunteers to support successful client relationships. Lead the Client Committee and engage clients in identifying needs and opportunities for connection and participation. Develop and maintain relationships with local agencies and community organizations that provide services to older adults. Plan and facilitate social activities and programs that promote connection, engagement, and quality of life. Manage social media communications and other outreach efforts to increase awareness of Y-OPAS services and community engagement. LEAD CHANGE Strengthen Services & Improve the Client Experience Identify opportunities to improve Y-OPAS services, client engagement, volunteer support, and community partnerships. Use client feedback and program information to identify trends, needs, and opportunities for improvement. Support innovative programs and activities that reduce social isolation and promote independent living. Contribute to volunteer recruitment, engagement, and retention strategies. Maintain current knowledge of community resources and services available to older adults. Support special projects and other program initiatives as assigned. REQUIREMENTS TO BE A YOPAS CLIENT COORDINATOR: Minimum age of 21. Bachelor's degree in social work, Gerontology, Human Services, or a related field preferred; two (2) years of relevant experience may be considered. Experience working with older adults, preferable in home or community-based settings. Knowledge of community resources and services available to older adults. Strong interpersonal, communication, organizational, and time-management skills. Ability to work independently and collaboratively. Proficiency with Microsoft Office or similar software; Canva experience preferred. Valid driver's license, reliable transportation, and required insurance with the ability to transport clients as needed. Ability to obtain and maintain an Arizona Fingerprint Clearance Card.
