PACE CENTER MANAGER

SUMMARY OF POSITION:The Center Manager for NEMS PACE (Program of All-Inclusive Care for the Elderly) program has the responsibility for a wide range of disciplines and services provided through the PACE Center, including all administrative responsibility related to direct operation of the PACE Center. The Center Manager coordinates overall operations of the PACE Center and serves as the liaison between direct care staff and administration.ESSENTIAL JOB FUNCTIONS:Coordinate all center activities including care planning and implementation, preparation for interdisciplinary conference and daily meetings, family conferences, and initial and period assessmentsSupervise clerical staff, social workers, transportation staff, recreational staff, dietary staff, and center aidesFacilitate and lead morning meetings of the interdisciplinary team (IDT)Facilitate and lead regularly scheduled Care Plan meetings to review participant cases on a regular basisOversee the quality of care and safety of PACE participants, both at the PACE center and in the communityServe as a liaison between disciplines and between administration and direct staffWork closely with the medical care team as part of the IDT to develop and implement participants’ individualized care plansCoordinate 24-hour care deliveryCoordinate with the marketing department to hold site tours for new candidates at the PACE CenterServe as a representative of NEMS PACE to the communityCoordinate activities related to new participantsManage the center’s budgetCommunicate with North East Medical Services administration on a regular basis about center operations and needsOversee overall documentation cohesiveness and accuracy in each participant’s medical recordUtilize Epic appropriately and efficiently and serve as content and workflow expert for resolving issues and proposing solutions in EpicPerform other job duties as required by supervisorADDITIONAL RESPONSIBILITIES:Work independently and as an effective member of a teamLead and facilitate IDT decision makingBe organized within busy center and clinic environmentsEstablish and maintain effective interpersonal relationships with all levels of staff, other departments, agencies, members, providers, and the general publicCommunicate effectively, verbally and in writing, with individuals from varying cultural backgroundsPrepare concise and clear reportsIdentify pertinent policy and operational issues; develop strategies, solutions, and recommendationsMulti-task in projects and their respective activities, timelines, and issuesUtilize computer and software toolsOnly act within the scope of his/her authority to practiceMeet a standardized set of competencies for the specific position description established by NEMS PACE and approved by the regulatory agencies before working independentlyBe medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contactPass a criminal background check which will verify that the candidate is: not excluded from participation in Medicare and Medicaid programs; has not been convicted of criminal defenses related to their involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act; and have not been convicted of physical, sexual, drug, or alcohol abuseBachelor’s degree in a health-related field required, Master’s preferred.At least 3 years of experience in a supervisory capacity within a healthcare setting.Minimum one year of experience working with a frail and/or older adult population preferred.Minimum one year of experience working with older adults and their families in a healthcare or community agency setting is preferred. Must be available to work any day of the week, Monday through Sunday, depending on need.Candidate must be comfortable working in a dynamic environment with direct care staff and administration staff.Excel in organizational and supervisory skills, with an ability to effectively handle difficult situations.Experience in successfully managing and completing projects is required.Experience with senior care clinical programs similar to PACE preferred.Experience working with clinical charts and records.Knowledgeable of the physical, mental and social needs of the elderly and be a professional in one of the following: Nursing, Social Work, Psychology, Recreation, Occupational Therapy, Physical Therapy, Speech Therapy, Dietetics, GerontologyCurrent BLS and First Aid requiredLANGUAGE:Must be able to fluently speak, read and write EnglishFluency in other languages are an assetSTATUS:This is an FLSA Exempt position.This is not an OSHA high-risk positionThis is a Full Time position.NEMS is proud to be an Equal Opportunity Employer welcoming diversity in our workforce.
