Job Responsibilities Assess patient’s home environment for safety hazards and architectural barriers, identify equipment needs. Identify functional problems and establish goals and a plan of care designed to improve the patient’s functional status (revising as necessary). Perform patient tests, measurements, and evaluations, such as range of motion and manual muscle tests, gait and functional analysis, and body part measurements, and record and evaluate findings to aid in establishing or revising specific treatment programs. Perform and complete start‑of‑care and ongoing assessments and associated paperwork in accordance with agency policy. Periodically re‑evaluate and upgrade the patient’s program to maximize functional improvement. Administer the exercise program utilizing specialized therapeutic exercise techniques and equipment. Plan and administer prescribed physical therapy treatment programs for patients to restore function, relieve pain and prevent disability following disease, injury, or loss of body parts. Recommend prosthetic/orthotic devices for patients or evaluate their devices and make minor adjustments or recommend modifications by the prosthetist/orthotist. Confere with the family, referring physician, and other agency professional staff to obtain information that promotes coordinated, efficient, and quality patient care. Perform and submit required documentation of patient evaluations, plan of care, clinical notes, and progress reports, summaries along with changes in orders. All patient visit notes must be turned in the next scheduled workday, not to exceed forty‑eight (48) hours. Strive to maintain positive internal and external customer service relationships. Demonstrate effective leadership skills. Communicate effectively by conveying necessary information accurately, listening effectively, and asking questions when clarification is needed. Plan and organize work effectively and ensure its completion. Demonstrate reliability by arriving to work on time and taking breaks within expected time frames. Meet all productivity requirements. Demonstrate team behavior and promote a team‑oriented environment. Represent the organization professionally at all times. Demonstrate initiative, continually improve processes and relationships. Education and Experience Graduate of an accredited Physical Therapy program and licensed by the state of Arizona. Previous experience in a clinical setting is preferred; motivated new graduates are welcome. Excellent communication and interpersonal skills. Ability to work both independently and collaboratively in a team setting. Strong commitment to patient‑centered care.
