Behavioral Health SpecialistJericho Road Community Health Center is actively seeking a Behavioral Health Specialist. The option for a flexible 40-hour workweek is possible within certain teams.Responsibilities:Maintain a caseload of approximately 30 one-hour patient sessions per week, dependent on program needsEngage patient in a healthy therapeutic relationshipAdvocate for patient needs when necessaryRefer patient to needed care within JRCHC or to outside resourcesProvide individual, group, or family therapyCollaborate with Providers, Interpreters, and Care Coordinators to ensure holistic careTake initiative with patients and Behavioral Health Team needsTo facilitate and triage referrals from various referral sourcesCoordinate skilled services for comprehensive case managementDemonstrate positive interrelationships and service excellence in the performance of duties by meeting or exceeding expectationsSupport the integration of Behavioral Health within a Medical ModelComplete tasks assigned by BH Director on an as needed basisMaintain confidentiality of records relating to clients' treatmentAdhere to the standards outlined in the Code of EthicsComplete necessary paperwork in a timely mannerSubmits service activity logs, weekly and monthly schedules, timesheets and mileage to the Behavioral Health Manager.Ensure licensure is in good standing, including upkeep of continuing education requirementsDevelop treatment plans for patientsComplete thorough patient assessmentQualifications:LMSW, LCSW or LCSW-R with a NYS License to practice, LMHC, or Licensed PsychologistStrong clinical skills in assessment, diagnosis and evidence-based therapeutic interventions.Experience working with diverse populations and individuals experiencing mental health challenges.Salaried Rate: $60,000 - $63,000 annually.
