We are seeking a compassionate and dedicated Registered Nurse to join our Hospice Team in the Media/West Chester, PA, and surrounding areas. The ideal candidate will have a strong clinical background and a passion for providing exceptional end-of-life care. Our team is committed to providing personalized support and comfort in the familiar surroundings of patients' homes.

Responsibilities

What You'll Do:

• Conducting start of care comprehensive assessments
• Drafting care plans, reconciling patient medications, and ensuring orders for care are accurate and complete
• Preparing accurate, orderly, and timely documentation
• Coordinating care with patients and providing proper reports to Case Manager
• Providing health care instructions to the patient/family as appropriate per assessment and plan

Qualifications

About You:

• An active RN license in the state in which practicing (Minimum of 1 year of experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Compassionate and empathetic approach to patient care
• Ability to work independently and as part of a team
• Knowledge of hospice regulations and standards of care
• Reliable transportation and willingness to travel
