Job Summary Position: Licensed Practical Nurse (LPN) / Registered Nurse (RN) Employment Type: PRN, Part Time, Full Time Schedule: Flexible Hours; Days, Nights, Weekdays, Weekends, Holidays Pay: $25.00–$30.00 per hour; Daily and Weekly Pay options available What You’ll Do Care for medically complex pediatric and/or adult patients in their homes. Provide skilled nursing care under RN supervision. Administer medications and treatments based on individualized plans. Educate and support families in in‑home care techniques. Observe and report changes in patient condition to RN and physician. Document visits using an easy‑to‑use EMR system. Participate in long‑term care team meetings and patient care conferences. Assist in preparing equipment and materials for treatments, observing aseptic technique. Help patients learn self‑care skills and home‑care techniques. Follow and implement the nursing care plan approved by RN/physician. Initiate preventive and rehabilitative nursing procedures as appropriate. Teach and counsel patients and families about nursing care needs and related issues. Evaluate, with RN, the effectiveness of your nursing services to patient and family. Maintain accurate records of observations, treatments, and care. Participate in medical record audits, peer review, utilization review, and QAPI activities. Attend staff meetings, patient care conferences, and in‑services as scheduled. Take on‑call duty during nights, weekends, and holidays as assigned. Submit any schedule changes to the scheduling position daily or as needed. Discuss the need for involvement of other health‑team members (physical therapist, speech‑language pathologist, etc.) at patient care conferences. Ensure patient/family involvement in implementing the plan of care. Assist in reviewing and updating patient care plans when appropriate. Provide timely and accurate documentation and paperwork as per agency policy. Immediately report/document incidents, variances, or complaints to Nursing Supervisor/RN. Demonstrate competence in technical skills before performing new ones. Maintain confidentiality and safeguard PHI in accordance with HIPAA requirements. Show effective communication with patient/family, team members, and other health‑care professionals. Adhere to state, federal, accreditation policies, and company standards. Maintain acceptable attendance status and clean, neat work environment. Show sound judgment, critical thinking, and decision‑making skills. Perform additional duties as assigned. What You Bring Active LPN license (with or without RN license) in the state(s) of practice. CPR certification (recertified annually). One year of nursing experience preferred; new graduates welcome. Excellent verbal and written communication skills. Strong critical‑thinking, problem‑solving, and decision‑making abilities. Ability to work independently and collaboratively within a team. Good computer skills and proficiency with EMR systems. Physical ability to perform duties, including lifting up to the required weight. Acceptance of the company's vision, philosophy, and goals. Comfortable working in a home‑health, long‑term care setting with pediatric and/or adult patients. Required Skills and Abilities Average computer skills with competency in media communications. Ability to execute technical procedures following established protocols. Effective time‑management to complete documentation and routine assignments. Demonstrated sound judgment and critical thinking during patient care. Compliance with HIPAA and privacy regulations. Physical Requirements Ability to stand, sit, walk, turn, and hold a patient for up to 30 minutes. Ability to transfer a patient (including the use of transfer equipment).
