The Outcomes-Based Care Manager is a customer-facing clinical expert who helps provider organizations shift from measuring care by hours delivered to measuring it by outcomes achieved.
You will work directly with customer clinical and executive leaders to implement outcomes frameworks, interpret progress data, and build the evidence that supports value-based and outcomes-linked payer relationships. You will use CentralReach's platform and AI-enabled tools to make that work scalable, and you will bring what you learn from the field back to our Clinical and Product teams. This is an individual contributor role reporting to the VP, Payor Strategy and Network Development.
