Roles & Responsibilities
- Provide skilled physical therapy services to residents in a skilled nursing facility
- Develop and implement individualized treatment plans to improve mobility, strength, and function
- Collaborate with an interdisciplinary team to ensure comprehensive patient care
- Educate patients and families on therapeutic exercises and home programs
- Document patient progress and adjust treatments as needed
- Promote a positive and healing environment for optimal recovery
Qualifications
- Valid Physical Therapist license in Connecticut (or compact license eligible)
- Graduate from an accredited physical therapy program (Master's or Doctorate)
- Minimum 1 year of experience in a skilled nursing or rehab setting
- Current BLS certification from the American Heart Association
- Strong clinical reasoning and problem-solving skills
- Excellent communication and teamwork abilities
Knowledge & Skills
- Expertise in evaluating and treating orthopedic, neurological, and geriatric conditions
- Proficient in electronic medical records and documentation
- Ability to work independently while being a collaborative team player
- Compassionate and patient-centered approach to care
- Time management skills to handle a diverse caseload
