Description

• Conduct evaluations for patients in either an inpatient or outpatient capacity and formulate tailored treatment plans

• Educate patients and their families about therapeutic objectives, discharge processes, and home exercise programs
• Maintain accurate documentation of evaluations, treatment plans, and patient progress consistent with facility and regulatory requirements
• Actively participate in care team discussions and interdisciplinary meetings to ensure synchronized patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• Preferred CPR certification
• Specialty certifications (e.g., neurological, geriatric) are advantageous, though not mandatory

Skills

• Prior experience is highly preferred
