Licensed Practical Nurse (LPN) - Primary Care Dayton, Ohio Position Responsibilities Provide direct and indirect nursing support in the primary care environment. Obtain patient history and support assessment and care planning. Document care and report changes in condition to the RN or physician. Monitor patient response to medications and therapies. Assist in emergencies and support diagnostic or procedural care. Review labs and medical records as part of patient support workflow. Provide patient and family instruction. Participate in discharge planning and team-based care functions. Maintain professional conduct, patient privacy, and customer service standards. Required Qualifications Graduate of an accredited practical nursing program. Current CPR certification from an American Heart Association vendor. Minimum one year of direct patient care experience within the last three years. VA experience preferred, not required. Ability to obtain and maintain VA credentialing and clearance requirements. Eligibility Requirements Permanent Resident/Green Card holder with a Social Security Number and must have resided within the United States for the past three years; No sponsorship available.
