Care Manager

Conduct home visits to implement care plans with clients and ensure a continued high level of care. Train and support caregivers in meeting their loved ones' needs. Monitor and report on clients’ health, coordinating with their care team and outside agencies as needed. Facilitate smooth transitions and ongoing care for clients. Maintain compliance with all relevant regulations and standards. Requirements Bachelor’s degree required; preferably in social work, human services, or a related field. LSW or LISW in state of employment preferred. At least one (1) year of experience working with elders or adults with disabilities. Ability to travel (by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required. Experience in case management, care implementation, planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting. Training, evaluating, and supervising caregivers. Documenting in an accurate and timely manner; maintain a clinical record for each patient. Proficiency in common software and web-based applications (e.g. MS Office, case management systems, EVV, etc.). Patient-centered approach with strong communication skills. Familiarity with New Hampshire Medicaid, CFI and/or caregiving ecosystem (including CMAs, government offices, and other stakeholders) is preferred.
