RN Hospice Case Manager

The RN Hospice Case Manager coordinates and provides compassionate, skilled nursing care to hospice patients in homes, assisted living facilities, and other settings, managing a caseload and collaborating with an interdisciplinary team to ensure high-quality end-of-life care. Key Responsibilities:

• Perform comprehensive hospice admissions and ongoing patient assessments
• Develop, implement, and revise individualized hospice plans of care
• Coordinate care with physicians, hospice aides, social workers, chaplains, therapists, and other members of the interdisciplinary team
• Provide skilled nursing care focused on pain management, symptom control, and patient comfort
• Educate patients and family members regarding disease progression, medications, hospice services, and end-of-life care
• Monitor patient status and adjust care plans based on changing clinical needs
• Coordinate necessary medical equipment, medications, and community resources
• Complete accurate and timely electronic documentation
• Participate in interdisciplinary team meetings and case conferences
• Ensure compliance with Medicare hospice regulations and agency policies
• Provide emotional support and advocacy for patients and their families throughout the hospice experience

Qualifications:

• Current Registered Nurse license in the State of Utah or eligible for licensure
• Graduate of an accredited nursing program with an ADN or BSN degree
• Minimum of one year of RN clinical experience required
• Strong clinical assessment and critical thinking skills
• Excellent communication, empathy, and interpersonal abilities
• Ability to work independently while managing a patient caseload
• Valid driver's license with reliable transportation
• Current auto insurance meeting state requirements
• Current BLS certification through the American Heart Association or American Red Cross

Preferred Skills:

• Previous hospice, home health, palliative care, oncology, critical care, or case management experience
• Experience with electronic medical records (EMR)
• Hospice and Palliative Credentialing Center (CHPN) certification
