The Opportunity

We are hiring a Registered Nurse (RN) on a per diem basis to support clients across Rhode Island. This is a morning (AM) role focused on in-home client assessments, care planning, and supervisory visits. The RN works under the direction of our Director of Nursing, who provides final clinical sign-off. Please note this position is assigned to non-skilled home care services only.

Key Requirement

• All applicants are required to pass a Competency Evaluation prior to being hired, in addition to meeting all listed requirements.

Responsibilities

• Complete initial client admissions and in-home assessments

• Develop and update individualized care plans for personal care services

• Conduct supervisory visits of Certified Nursing Assistants and homemakers at least every 60 days, or more often as the client's condition requires

• Document all visits accurately and on time in Axxess

• Coordinate with the Director of Nursing and the care team on client needs and condition changes

• Adhere to all applicable Rhode Island state regulations governing home care

Requirements

• Active Registered Nurse (RN) license in Rhode Island

• Current BLS certification (ACLS a plus)

• Clinical experience in a hospital, facility, or home care setting

• Reliable transportation for in-home visits across Rhode Island

• Strong documentation and communication skills

• Available to work mornings on a per diem basis

• Bilingual in Spanish and English is valued but not required

Pay Structure

• Per-visit compensation for initial assessments, admissions, and supervisory visits

• Compensation is processed upon submission and completion of client documentation

Note

The initial in-home assessment must be conducted within forty-eight (48) hours. Supervisory visits of the Nursing Assistant (at least one supervisory session) must be conducted no less than every sixty (60) days thereafter, or more frequently as the client's condition requires.
