Body and Mind Medical Center, an adult primary care practice in Chicago’s Wicker Park neighborhood, is seeking a compassionate, dependable part-time nurse practitioner to provide patient care both in the office and online. This role involves evaluating, diagnosing, and managing common and chronic conditions in an adult patient population, offering follow-up care, and educating patients. The position features a flexible schedule with a mix of in-office and telehealth appointments, requiring strong communication skills and a patient-centered approach.

Responsibilities

• Conduct patient evaluations and assessments
• Diagnose and treat common adult health conditions
• Manage chronic diseases and ongoing care plans
• Provide timely follow-up care and patient monitoring
• Educate patients about health management and treatments
• Coordinate patient care with other healthcare providers
• Deliver telehealth services efficiently and effectively
• Document patient visits accurately in medical records

Preferred Qualifications

• Entry-level experience in nurse practitioner practice
• Proficiency in patient assessment and clinical diagnosis
• Experience managing chronic diseases
• Familiarity with electronic health records (EHR) systems
• Comfortable using telehealth technology
• Strong patient education and communication skills
• Empathy and a compassionate patient care approach
