REQUIREMENTS
- 2+ Years of Hospital Social Working Experience
- Discharge Planning Experience
- Experience with All Patient Populations
- LSW in the State
- Epic Experience

ROLES & RESPONSIBILITIES
- Provide services to customers and their families to help them adjust to illness, injury, or hospitalization.
- Assist the health team in understanding the significance of social, economic, emotional, and family factors in relation to the customer's illness, treatment, recovery, or dying processes.
- Facilitate timely, efficient, and appropriate discharge of customers and coordinate with the next level of care, including transmission of the appropriate plan of care.
- Identify needed community services and coordinate appropriate services for customers in need.
- Participate in and implement discharge planning services for customers, including screening and coordinating referrals for nursing homes, assisted living facilities, hospice, and more.
- Assess customers' and families' risk factors and provide interventions as needed, including addressing issues related to child abuse, guardianship, and mental health placement.
