This position focuses on senior patients within a value-based care model, with smaller patient panels, dedicated care-team support and an emphasis on continuity of care and long-term patient outcomes. Physicians care for seniors who commonly have multiple complex chronic conditions, including heart disease, diabetes and hypertension.
