Job Responsibilities:
Provide patient-centered care to individuals in their homes
Assess patients' functional abilities and develop personalized treatment plans
Implement therapeutic activities and exercises to improve patients' independence and quality of life
Educate patients and their families on techniques for managing daily activities
Collaborate with other healthcare professionals to ensure comprehensive care for patients
Qualifications:
Valid Occupational Therapist license in the state of Nebraska
Experience working with patients in a home healthcare setting
Strong interpersonal and communication skills
Ability to work effectively in a team environment
Passion for providing patient-centered care
