Program Clinical Coordinator Case Manager

Program Clinical Coordinator (Case Management) Program Name: California MENTOR Family Home Agency Office Location: Alhambra Program Area: East LA, Whittier, Pico Rivera, Montebello (travel required) Pay: $45,800 annual salary (overtime eligible) Responsibilities Coordinate development, review, and implementation of services, case management, behavior intervention plans, and treatments for individuals served. Oversee all services provided to individuals and their families, monitor quality of care delivered, ensure compliance with contractual terms, and ensure regulatory program compliance. Assist in the development of quality services and engaging activities that meet individuals’ needs. Build and maintain relationships with families and external case managers. Qualifications Bachelor’s degree in human services or a related field. One year of work‑related experience with individuals with intellectual and developmental disabilities. Valid California driver’s license and reliable vehicle with proof of registration and insurance. Reliable, responsible, compassionate approach and commitment to quality. On‑call availability after hours and on weekends. Applicants without a bachelor’s degree will be considered if: High school diploma or GED. At least two years of experience in a similar program serving individuals with developmental disabilities. Benefits Full‑time position, schedule Mon‑Fri 8:30am‑5:30pm.
