JourneyCare is seeking a dedicated Registered Nurse (RN) to serve as a Hospice Clinical Liaison supporting hospital-based referrals in Evergreen Park, Illinois. This full-time position involves travel throughout the south side of Chicago and collaboration with interdisciplinary teams. The ideal candidate will have 1–2 years of RN experience, strong clinical judgment, and excellent communication skills.
