RN Case Manager - Acute Care & Discharge Planning

Universal Health Services is hiring a Case Manager to support Southwest Healthcare Inland Valley Hospital in Wildomar and Rancho Springs Hospital in Murrieta, CA. The role focuses on assessing patient needs, coordinating discharge planning, and applying sound clinical judgment within the nursing process.

The ideal candidate has 3+ years of acute care experience, RN licensure in California, and exposure to Case Management or Utilization Review. CCM/CPUR certifications are preferred.
