MDS Coordinator (RN or LPN)Outfield Healthcare Partners is seeking a skilled MDS Coordinator (RN or LPN) to relocate to one of our New Mexico skilled nursing facilities. This role is responsible for accurate and timely completion of the RAI/MDS process, care planning coordination, and regulatory compliance from admission through discharge.
