JOB SUMMARY

Coordinates outreach education and care coordination services to engage patients in preventive and ongoing healthcare support closure of care gaps improve clinical outcomes and promote effective use of healthcare resources. Assists patients in addressing clinical and non-clinical needs by facilitating access to community resources supporting treatment adherence and self-management and helping remove barriers to achieving care plan goals.

JOB DETAILS AND REQUIREMENTS

Type: Full Time (80 hours per 2 week pay period with benefits)

Typical hours for this position: Monday-Friday 8:00am-4:30pm

Education:

- High School diploma or GED- required

License/Certification:

- BLS- required

- Community Health Worker certification- preferred

Experience:

- Previous CHW or care coordination work with community resources- preferred
