Licensed Practical Nurse (LPN) – Weekend Private Duty (Friday 5 PM – Monday 8 AM) KeystoneCare is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide private duty nursing care for a patient with ALS in their home. This role is ideal for a nurse who is committed to delivering high-quality, patient-centered care while supporting both the patient and their family. This position involves extended weekend coverage from Friday at 5:00 PM through Monday at 8:00 AM , providing continuity of care and ensuring the patient’s safety, comfort, and well-being. Primary Job Responsibilities Provide skilled nursing care in accordance with physician orders, patient care plans, and agency policies Administer medications and treatments as prescribed Perform ongoing patient assessments and monitor for changes in condition Ensure patient safety, comfort, and overall well-being throughout the shift Communicate effectively with patients, families, and interdisciplinary care teams Educate patients and families on care plans, equipment use, and safety measures Maintain accurate and timely documentation in compliance with agency standards Job Requirements Graduate of an accredited LPN program Current LPN license in Pennsylvania Minimum of one year experience in home healthcare or a similar clinical setting Familiarity with Feeding Tubes, Assisted breathing Devices (Ventilators) and Hoyer Lift CPR certification Strong communication and interpersonal skills Self-motivated with the ability to work independently Reliable transportation and valid Driver's License Benefits Flexible scheduling options Paid Time Off Travel reimbursement between work sites Paid Holidays for Full-time staff Medical, Dental, Vision, and Aflac plans available for Full-time staff $25K Company Paid Life Insurance Policy for full-time staff Retirement plan with discretionary employer match #J-18808-Ljbffr KeystoneCare
