Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor‘s, Master‘s or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• WA license information and any other needed information for credentialing must be in place at a minimum within 10 days of start if not already Medicare/Medicaid credentialed
