Help patients begin their recovery at home. As an Admissions RN with Elara Caring, you’ll make a meaningful first connection with patients and families—assessing their needs, explaining their care, and helping set up the services and support they need to move forward. Your responsibilities will include: Conducting initial nursing assessments and evaluating patients’ physical, psychosocial, and environmental needs Completing admission and start-of-care documentation, including OASIS assessments as applicable Developing and updating individualized plans of care in collaboration with the patient, family, physician, and care team Explaining home health services, care plans, and next steps to patients and caregivers Identifying changes in a patient’s condition or safety concerns and communicating them promptly to the appropriate team members Coordinating with the Clinical Supervisor, physicians, and interdisciplinary team to support a smooth transition into home health Maintaining accurate clinical documentation and protecting patient privacy in accordance with HIPAA What You’ll Need: Current, unrestricted RN license in the state of practice One year of nursing experience in a clinical care setting Willingness and ability to travel throughout the branch’s coverage area Dependable vehicle, valid driver’s license, and current auto insurance as required by state law Ability to frequently lift, push, pull, and support up to 50 pounds, including when assisting with patient transfers or moving equipment Associate or bachelor’s degree in nursing Home health or hospice experience preferred OASIS experience preferred Homecare Homebase experience preferred Equal Employment Opportunity : We are proud to be an equal opportunity workplace and comply with state and federal affirmative action requirements.
