About the Position

Job Title: Physical Therapist Profession: Physical Therapist Specialty: Rehab Duration: 13 weeks Shift: Days Hours per Shift: 8 hours M-F, flexible times Experience: Minimum 1 year of PT experience, LTC or Rehab experience License: Missouri State License Certifications: N/A Must-Have: New Grads OK if they have Level II experience Description: Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care. Develops appropriate plans of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Administers various procedures as part of the rehabilitation plan including but not limited to manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices. Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness and modifies services based on patient evaluation. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services provided per regulatory and clinical requirements. Documents all regulatory and clinically required information in the electronic documentation system. Maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements. Participates in required meetings including patient care conferences and rehabilitation meetings. Contributes to facility and company-required trainings, in-services, and conferences. Able to obtain any necessary durable medical equipment or supplies per regulatory and clinical practice requirements. Supervises and supports physical therapist assistants, rehabilitation aides, and/or students. Adheres to practice acts, codes of conduct, and ethics during professional practice as a Physical Therapist.
