What You'll Need to Succeed:

• Proven experience evaluating, screening, and rehabilitating patients with physical functioning disorders and wound care
• Strong knowledge of clinical assessment tools and evidence-based practice
• Ability to develop and implement personalized care plans
• Skilled in administering therapeutic procedures including manual techniques, ambulation, exercises, modalities, supportive devices, and wound management
• Excellent communication skills for collaborating with patients, families, caregivers, and healthcare team members
• Proficiency in documenting patient progress and treatment outcomes accurately in electronic health records
• Ability to supervise and support rehabilitation aides, students, or assistants
• Adherence to professional standards, ethics, and regulatory guidelines (APTA, state and federal laws)

Nice to Have Skills:

• Experience with DME (Durable Medical Equipment) procurement and supply management
• Participation in multidisciplinary team meetings and professional development training
• Bilingual abilities or additional language skills to support diverse patient populations

Preferred Education and Experience:

• Bachelor's or Master's degree in Physical Therapy (DPT preferred)
• Valid state licensure as a Physical Therapist
• Prior experience working in long-term care or skilled nursing facility settings

Other Requirements:

• Willingness to work weekdays in a day shift schedule
• Reliable transportation for potential facility visits and supplies procurement
• Certifications in CPR and Wound Care (preferred but not mandatory)
