Healthcare Coordinator LPN

The role of the Healthcare Coordinator is to assist in achieving CSI's mission by ensuring that the individuals supported by CSI receive high quality health services. This includes ensuring early evaluation of health/dental needs, timely implementation of required treatment and the review of CSI policies and/or procedures to assure that the services rendered meet or exceed all applicable standards of care. This is an on-site only position in the Plymouth/Cape Cod area. Position pays $35/hour with rotating on-call availability on the weekends.

Essential Functions, Duties, and Responsibilities:

• Provide overall healthcare coordination to persons supported in their own homes.
• This includes establishing health profiles, identifying health risks and ensuring that persons supported have the opportunity to choose healthy lifestyles.
• Provide leadership, guidance and training for staff to effectively maintain the health status of persons supported and to ensure accurate and timely implementation of medical orders.
• Oversee the creation and maintenance of persons' medical records to ensure compliance with state and internal regulations and policy.
• Participate as a member of interdisciplinary team to advocate for the persons supported.

Active licensure in the state of Massachusetts as an LPN. At least two years recent nursing experience in an acute or long-term care setting. Experience working with individuals who have intellectual disabilities preferred. Ability to work independently and manage a flexible work schedule. Excellent written and oral communication skills, computer/word processing skills. Valid U.S. vehicle driver's license.

Work Environment:

The Healthcare Coordinator LPN's environment may vary widely each day. He/she may function within a person's home, including the kitchen and specially equipped bath. They will also reside in the business office within the nursing office space. Hazardous material exposure is generally limited to household items such as cleaning items. The duties may present an exposure to bloodborne pathogens.

Required Trainings:

Employees are required to be certified in CPR, First Aid, Physical Management techniques (or the equivalent), Medication Administration, Universal Precautions and other OSHA regulations. They must complete and maintain renewals in all trainings listed in the CSI Policy Manual as mandatory trainings. In addition, there are several county or state-specific training requirements state to state. Such training certificates shall be obtained within the timeframes mandated by CSI policy and the counties and states on whose behalf CSI manages programs. Staff must also be trained in each person's individualized support plans. Training may require formalized testing.

Job Qualifications:

The minimum requirements are a 2 year degree, minimum age requirement 20 years, functional literacy in math and reading, and a valid driver's license. Candidates must also have a satisfactory criminal, driving, and employment history per the CSI Personnel Policy. An Associate's degree in a related field of study is preferred.

Sufficient dexterity to operate standard business equipment and other specialized equipment. Vision for everyday reading and the ability to drive. Ability to walk, sit, stand, and climb stairs. The ability as necessary to physically restrain individuals who could be a risk of danger to themselves or others. Reach and bend at the waist, kneel on the floor and twist without restrictions.
