Physical Therapist

The Physical Therapist (PT) provides skilled physical therapy services within the legal scope of practice, delivering individualized care plans and interventions to promote patient mobility, independence, and safety in home health settings. The role includes conducting evaluations, reassessments, and collaborating with interdisciplinary teams while ensuring compliance with regulatory standards and documentation requirements. Responsibilities:

• Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications following Medicare, state, and agency standards.
• Develop, implement, and update individualized plans of care aligned with physician orders and patient needs.
• Deliver skilled interventions and maintenance therapy programs as permitted by state-defined scope of practice.
• Directly supervise Physical Therapist Assistants (PTAs) and Home Health Aides in compliance with state practice acts.
• Consult with physicians and interdisciplinary team members regarding treatment adjustments and patient progress.
• Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies.
• Prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges within agency-defined timeframes.
• Apply ICD-10 coding accurately for compliant documentation.
• Present in-service programs and contribute to staff education initiatives.
• Engage actively in Quality Assessment and Performance Improvement (QAPI) activities to support clinical excellence.
• Maintain infection control, safety, and risk prevention protocols during care delivery.
• Document clinical interventions, care coordination, and communication timely and accurately.
• Participate in interdisciplinary meetings, case reviews, and continuing education opportunities.
• Maintain compliance with HIPAA and organizational privacy standards.
• Represent the organization professionally in patient homes and community settings.
• Work independently with minimal oversight, demonstrating initiative and adaptability.
• Provide courteous, timely, and professional customer service to patients, families, and partners.
• Utilize organizational technology systems effectively for communication, documentation, and data management.
• Adhere to all organizational policies, including mission and employee handbook guidelines.
• Perform additional duties as assigned to fulfill role responsibilities.

• Graduate of an accredited physical therapist curriculum approved by recognized accrediting bodies.
• Current Physical Therapist license in the state of practice.
• Basic computer proficiency with willingness to learn EMR systems and OASIS documentation.
• Current CPR certification.
• Reliable transportation, valid driver's license, and automobile insurance for home visits.
• Ability to exercise sound clinical judgment and work independently.

• Two years of clinical experience preferred.
• Home health experience is a plus but not required.
• Strong communication and interpersonal skills with sensitivity to diverse backgrounds.

• Must comply with Medicare/Medicaid Conditions of Participation, state regulations, and agency policies.
• Work involves providing care in patients' homes and community settings requiring professional representation of the organization.
• Must adhere to infection control, safety, and risk prevention protocols.
• Ability to work independently with minimal daily oversight and adapt to changing responsibilities.

• Work schedule and shift assignments are determined based on patient needs and agency requirements, typically involving home visits during standard business hours.
