About the Role:

• Provide direct patient care, including administering medications, wound care, and vital signs monitoring, under the supervision of a Registered Nurse (RN) or a licensed physician.

• Develop, implement, and evaluate individualized care plans for patients with various medical conditions and treatment needs.

• Collaborate with healthcare teams, including physicians, therapists, and other LPNs, to ensure seamless patient care transitions and optimal outcomes.

• Document patient care and progress accurately and thoroughly, in accordance with facility policies and regulatory requirements.

• Participate in patient education and discharge planning to ensure patients receive the necessary skills and support for successful transitions to home or other care settings.
