Social Worker - CHC

Liquid Personnel is recruiting an Adult Social Worker to join its client’s CHC team, based in Somerset. This is a key opportunity to contribute to high-quality Continuing Healthcare (CHC) assessments and support robust, evidence-based decision making within a multi-disciplinary environment. What will your responsibilities be? Complete comprehensive CHC assessments across a range of care settings in line with the National Framework. Lead and contribute to Decision Support Tool (DST) processes with accuracy and sound professional judgement. Act as a core member of the multi-disciplinary team, representing the local authority perspective. Provide constructive challenge to ensure consistent and lawful CHC eligibility decisions. Apply in-depth knowledge of the Care Act 2014, including Section 22 and the limits of local authority provision. Explore and support appropriate joint funding arrangements where applicable. Qualifications and Experience To be successful in this role you must have; Qualified Social Worker with current professional registration. Extensive experience completing CHC assessments and DSTs. Strong understanding of the CHC National Framework. Detailed knowledge of the Care Act 2014, particularly Section 22. Excellent communication, analytical and stakeholder management skills. Ability to work effectively within a multi-disciplinary team.
