Housing Stability Navigator - Community Support
BMC Software Inc
Boston Medical Center’s Living Well at Home Program (LWAH) expands services through a new Community Support Program for Homeless Individuals. The Housing and Community Support Specialist will engage high-risk patients, facilitate enrollment in services, and help them access and stabilize independent housing with a patient-centered approach. The role requires a Bachelor’s degree or equivalent experience, and supports a hybrid schedule with community visits across Greater Boston. #J-18808-Ljbffr BMC Software
