Remote RN Care Manager: Telehealth & Care Coordination

Welby Health is seeking a licensed RN to serve as Care Manager for our remote physiological monitoring program. You will monitor vitals, develop care plans, and provide education to improve adherence and outcomes.

In this remote role, you will collaborate with physicians and cross-functional teams, coordinate care, and empower patients and families with guidance and coaching via secure messages. A Michigan RN license and Spanish proficiency are required, with Illinois or Georgia licenses a plus.
